Locum Social Worker – Hospital Discharge to Assess (South Team)
Manchester City Council

Are you an experienced and dedicated Social Worker looking for a rewarding locum opportunity in a dynamic and supportive team? Manchester City Council is seeking a committed professional to join the Hospital Discharge to Assess South Team working through Service Care Solutions.

As part of this fast-paced and vital service you will:

Support hospital discharges assessing and reviewing the care needs of citizens transitioning out of hospital.
Complete Care Act assessments and Mental Capacity assessments as required.
Work across all hospital discharge pathways including:

Reviewing care packages in the community
Discharges from intermediate care beds
Citizens discharged to Pathway 3 (e.g. residential and nursing care settings)
Contribute to crisis response work helping prevent unnecessary hospital admissions.
Collaborate with the Hospital at Home service to ensure safe timely discharges and community support.
Participation in a 1-in-4 weekend rota required (supportive team environment).
Flexible hybrid working considered following induction and service need.✅ Requirements:
Qualified Social Worker (SWE registered)
Experience in hospital discharge / adult social care settings
Strong working knowledge of the Care Act, MCA and hospital pathways
